How to Add a File Based Question to a Form

Overview

A File upload question lets learners attach a file as their answer — for example "Please upload your certificate". You can make it a required field so learners can't submit without providing the file. It's one of the question types you can pick when building a Form or quiz.



Add a File upload question

  1. Open the Form you want to edit — you'll land in the question builder in edit mode.
  2. Click Add question at the bottom of the builder to add a new question.



  1. With the new question selected, open the question type dropdown to the right of the question title and choose File upload.



  1. Type your question in the question field. You can add an Additional description (optional) in the text box below to explain what learners should upload.
  2. Use the Required toggle if learners must attach a file. Your changes save automatically as you work.



Good to know: Use the Additional description field to spell out exactly what you need — for example the accepted file type or what the document should show.


Things to keep in mind

  • Learners answer by uploading a file, so there are no answer options to set.
  • File upload questions record the uploaded file as a response and don't count towards a Form's pass score.
  • Changes to a Form save automatically as you work — there's no separate save button.


FAQs

Can I make the file upload required?
Yes. Turn on the Required toggle so learners can't submit the Form without attaching a file.


Is a File upload question graded?
No. The uploaded file is recorded as the learner's response but isn't auto-marked, so it doesn't contribute to a Form's pass score.


Who can do this

Owners, Admins, Managers, Content Managers and Instructors can add and edit Form questions. People Managers, Employees and Restricted users have read-only access and cannot change a Form's questions.
