Overview

Forms let you survey your team; quizzes let you test their knowledge. The difference is simply whether you mark correct answers and set a pass score. This article covers building a form from scratch, adding questions, and marking the correct answers on multiple-choice and checkbox questions so SuperPath can grade responses automatically.

How to create a form or quiz

  1. In the left-hand menu, go to Forms & Quizzes.
  2. Click Create Blank Form to start from an empty form. (You can also click Auto Generate Form to have SuperPath draft a quiz for you — see the AI Forms article.)
  3. SuperPath opens the new form in the builder, ready for you to add a title and questions. Changes save automatically as you go.

Good to know: A "form" and a "quiz" are the same thing in SuperPath — a form becomes a quiz once you mark correct answers and set a pass percentage (covered below).

How to add questions

  1. In the form builder, click Add question to add a blank question. New questions default to a Multiple choice question.
  2. Alternatively, click Add question from template and choose a ready-made question from the Select a question template list — for example Multiple choice - True or False, Multiple choice - Yes or No, or one of the rating templates.
  3. Type your question into the question title, and add your answer options.
  4. Use the question type selector on the right of a focused question to change the type. The available question types are: Multiple choice, Checkboxes, Open text answer, File upload, Rating, Date and Time.
  5. Use the toggles at the bottom of a question to mark it Required, and use the copy or delete icons to duplicate or remove a question.

Note: Only Multiple choice and Checkboxes questions can be graded automatically. Open text, file upload, rating, date and time questions are recorded as responses but are not auto-marked as correct or incorrect.

How to set the correct answers

On a Multiple choice or Checkboxes question, each answer option has a small tick box under the Answer column heading on the right.

  1. Click into the question so it is focused (editable).
  2. For each answer option, tick the box under Answer to mark that option as correct.
  3. On a Multiple choice question, mark the single correct option. On a Checkboxes question you can mark more than one option correct — the learner must select exactly the correct set (no more, no less) to get the question right.
  4. There is no separate save button — the correct answer is stored as soon as you tick it.

Tip: For a question to be graded, it must be marked Required and have at least one correct answer ticked. Questions without a ticked correct answer are treated as ungraded and always count as "correct" when scoring.

After a learner submits, correct answers show a green tick and incorrect answers show a red cross, with the message "The correct answer is …" revealed for questions they got wrong (unless retries are switched on).

How to turn your form into a graded quiz

Marking correct answers tells SuperPath how to grade each question, but the form only becomes a scored quiz once you set a pass percentage:

  1. In the form builder, click the Settings cog next to the form title to open the Form settings panel.
  2. Turn on the Pass percentage (%) toggle ("Learners must achieve this score to pass").
  3. Enter the percentage a learner needs to pass, between 0 and 100.
  4. Optionally turn on Allow Retries and set the No. allowed attempts so learners can retake the quiz.

For more on pass marks, retries and how failing affects progression, see How to Require a Passing Score to Progress Past a Quiz.

Things to keep in mind

  • A form is only graded when its Pass percentage is greater than 0. With no pass percentage set, responses are still collected but no score is calculated.
  • Only Required multiple-choice and checkbox questions that have a correct answer ticked count towards the score.
  • Changes to a form save automatically — there is no separate "publish" step inside the builder. Assign the form (or add it to a Pathway) when you are ready for learners to take it.
  • Correct-answer feedback shown to learners after submitting is hidden while Allow Retries is on, so learners aren't shown the answers between attempts.

FAQs

What's the difference between a form and a quiz?
Nothing structural — they use the same builder. A form becomes a quiz when you mark correct answers on its questions and set a Pass percentage so responses can be scored.

Can a checkbox question have more than one correct answer?
Yes. On a Checkboxes question you can tick several options as correct. The learner must select exactly those options — selecting too few or including a wrong option marks the question incorrect.

Where do I mark the correct answer?
On a focused multiple-choice or checkbox question, tick the small box under the Answer column beside the option you want to mark correct.

Do open-text or file-upload questions get graded?
No. Those question types are recorded as responses but are not auto-marked. You can add a model answer to an open-text question to show the expected response after submission, but it does not affect the score.

Who can do this

Owners, Admins, Managers, Content Managers and Instructors can create forms and quizzes, add questions and mark correct answers (they hold create/manage permission on Forms and Questions). People Managers, Employees and Restricted users have read-only access to Forms and cannot build or edit them — the Forms & Quizzes area and the Create Blank Form button do not appear for them.
