Overview
Forms are a great way to survey your team or to test what they've learned from a course or Pathway. A Form and a quiz use the same builder in SuperPath — a Form becomes a quiz once you mark correct answers and set a pass score. Create a Form
- In the left-hand menu, go to Forms & Quizzes.

- On the Forms & Quizzes list, click Create Blank Form. (You can also click Auto Generate Form to have AI draft a quiz for you — see How to Use AI Forms to Instantly Generate Quizzes.)

- SuperPath opens the new Form in the builder. Give it a title and description in the text fields at the top.

- Add questions to your Form using either of the add-question options in the builder.

- There's no separate save button — every change you make to the Form saves automatically as you work.

Good to know: A Form and a quiz are the same thing in SuperPath. Your Form becomes a graded quiz once you mark correct answers on its questions and set a pass percentage.
Things to keep in mind
- Changes save automatically — there's no publish step inside the builder. Assign the Form (or add it to a Pathway) when you're ready for learners to take it.

- You can start from scratch with Create Blank Form, or let AI draft one for you with Auto Generate Form.
FAQs
What's the difference between a Form and a quiz?
Nothing structural — they use the same builder. A Form becomes a quiz when you mark correct answers on its questions and set a pass percentage so responses can be scored.
Do I need to save my Form?
No. Changes save automatically as you build, so there's no separate save button.
How do I add questions?
Once the Form is open in the builder, use the add-question options to add a question. New questions start as Multiple choice by default, and you can change the type per question.
Who can do this
Owners, Admins, Managers, Content Managers and Instructors can create Forms and quizzes (they hold create/manage permission on Forms). People Managers, Employees and Restricted users have read-only access to Forms and don't see the Forms & Quizzes area or the Create Blank Form button, so they can't create Forms.
