How to Create a Form
The purpose of this article is to guide users in creating a form in SuperPath. Forms are a great way to conduct surveys for your team or administer a test to assess if they have learned anything from the course or pathway.
To create a Form follow the steps below:
STEP 1 - On the left hand side, you will find the Library, click on it and it will show three (3) different choices. Click on Forms & Quizzes.
STEP 2 - After clicking Forms & Quizzes, it will bring you to the Forms & Quizzes library, where you can click "Create Form".
STEP 3 - Once clicked, you will see a text bar where you can put the title of the form and the form description.
STEP 4 - You can now add questions to your forms by clicking either of the options below.
STEP 5 - Every time you make a change to the form, your work will automatically save.
