Overview
Once learners start completing a Form or quiz, you can review what they answered, see how a graded quiz was scored per question, and export the results. Response reporting is per-form and answers the question "what did people answer?" — which is different from completion reporting, which answers "who has finished?". This article covers both, and the two export options.

How to view responses to a form
- In the left-hand menu, go to Forms & Quizzes.
- Find the form you want in the list (use the search box if you have many).
- In the form's row, click the View responses icon (the list icon in the row actions).
- SuperPath opens the form's Responses view.
If no one has completed the form yet, you'll see "We haven't received any responses yet" instead.

Summary vs Individual views
The Responses view has two tabs:
- Summary — an aggregated view across all respondents. Each question shows how people answered: multiple-choice and rating questions as charts, checkboxes as a bar chart (with an average for ratings), and open-text or file questions as a list of responses with a per-question response count.
- Individual — one respondent's submission at a time. Use the name dropdown to switch respondent. If a learner made several attempts, a View submissions dropdown lets you pick which attempt to view.
For a graded quiz, each answer in the Individual view is marked correct (green tick) or incorrect (red cross) in the status column, so you can see exactly which questions a learner got right.

How to export responses to PDF
The PDF export covers one respondent's submission:
- On the Responses view, switch to the Individual tab.
- Select the respondent (and, if there are several, the attempt) you want.
- Click the Download PDF button (the PDF icon on the right of the tab bar). It shows "Generating…" while it prepares.
- SuperPath downloads a PDF named after the form and the learner.

Note: The Download PDF button only appears on the Individual tab once a respondent is selected, and it exports that respondent's selected submission — not everyone's responses at once.
How to export all responses to CSV
To export every response across all respondents:
- On the Responses view (either tab), click Export All (the cloud-download icon on the right of the tab bar).
- SuperPath downloads a CSV file built from the loaded responses.
The CSV has one row per answer and includes columns such as Question, Response, Learner full name, Question type, Date created, and — useful for quizzes — Is correct, alongside the learning, question and user IDs.
Good to know: The CSV is generated in your browser from the responses already loaded on screen, so it reflects the current form's responses.
Responses vs completion reporting
These two are reached from different icons on the Forms & Quizzes list and answer different questions:
- View responses (list icon) — the Responses view described above: the actual answers people gave, per-question correctness, and the Summary/Individual breakdown.
- View learners (bar-chart icon) — the completion report for the form: who it's assigned to, their status and percentage complete, and (for a graded quiz) a Score column on screen. This report has its own Export button that downloads a learner report CSV (names, email, status, assignment and completion dates, and so on).
Use View responses when you care about the answers; use View learners when you care about who has completed it.
Things to keep in mind
- The PDF export is per-respondent (one submission); the CSV Export All covers all responses to the form.
- Quiz score per question (correct/incorrect) appears in the Responses view and in the responses CSV's Is correct column. The overall numeric score shows on screen in the View learners completion report, but is not included in that report's CSV columns.
- Responses to questions that have since been deleted from the form are filtered out of the Responses view.
- Form and quiz responses always identify the respondent by name — there is no anonymous-responses option.
FAQs
Can I export everyone's responses in one file?
Yes — use Export All to download a CSV containing every response to the form. The PDF export, by contrast, is one respondent's submission at a time.
Can I see which questions a learner got right?
Yes. On the Individual tab, each answer is marked correct or incorrect, and the CSV includes an Is correct column.
Where do I see who has and hasn't completed the quiz?
Use the View learners icon on the Forms & Quizzes list — that opens the completion report with status and progress, separate from the Responses view.
Can learners respond anonymously?
No. Responses are always attributed to the learner who submitted them; there is currently no anonymous option.
Who can do this
Owners, Admins, Managers, Content Managers and Instructors can open the Forms & Quizzes area, view responses and use the PDF and CSV exports. People Managers, Employees and Restricted users have read-only access to Forms and do not get the Forms & Quizzes area, so they cannot open the Responses view.
